Payment Policies & Customer/Therapist Responsibilities

Please Review all Policies Before Agreeing to Services

  1. If you will be utilizing commercial/ state insurance (Medicaid/Medicare) please provide policy information before the start of services. All insurance will be verified for co-payment cost before the start of services. This will be reviewed before the start of services. (Please note: The therapists are responsible for communicating copayment cost).
  2. A credit/debit card must be on file before the time of your initial appointment. Your co-payment must be rendered during your scheduled appointment time to avoid processing issues. If you are not able to pay your co-payment during the appointment time, please schedule a payment arrangement in which a co-payment amount can be automatically drafted from your account. (Please note: If you have more than five (5) unpaid co-payments you will not be able to schedule another appointment until the outstanding payments are satisfied.) We are here to help; please work out payment arrangements with the office manager.
  3. Sliding scale rates are offered and worked out individually with clients based upon their annual/gross income. (Please note: That all payments must be made at the time of the appointment.) If the payment is not made during the time of the appointment the scheduling will be hindered until payment is made. The credit card must be on file; or the other payment options listed below are acceptable if communicated. (Important note: Proof of income must be provided to make determination of sliding scale costs).

  4. Late cancellation policy: If an appointment is cancelled in less than 24 hours/or same day there will be a $50.00 late cancellation fee before the scheduling of the next appointment.
  5. Depending on your insurance you may have a deductible to pay before your insurance covers the pricing of the therapy sessions (e.g., if your deductible is $2,000) you will have to pay $2,000 dollars before you will be able to have your insurance cover the cost. This can be done over the course of several sessions until you meet the amount of your deductible. Insurance will be run prior to the start of services to alert you if you have a deductible.
